The American Academy of Osteopathy Louisa Burns Osteopathic Research Committee (LBORC) developed the Outpatient Osteopathic SOAP Note Form Series as part of a grant from the American Osteopathic Association. These forms are valid, standardized and easy to use. The LBORC highly recommends these forms to the osteopathic medical profession for research and training in osteopathic medicine. The LBORC provides training for osteopathic physicians, residents, interns and students who may be interested in using the Outpatient Osteopathic SOAP Note to record data on their patient encounters to support research on osteopathic principles and practices. Physicians who complete this training and are certified can contribute their own data to a national osteopathic clinical database of patient records. The aggregate data will be made available to osteopathic researchers to document the clinical outcomes and efficacy of OPP and osteopathic manipulative treatment. This record keeping system could also provide for improved financial reimbursement and an easy means of educational tracking of patient encounters.
